The NMC publishes per-endorsement Record of Assessment sheets documenting hands-on demonstrations of competence for STCW endorsements (such as OICNW, OICEW, Able Seafarer-Deck, and RFPNW). Each assessment is signed by a Coast Guard-approved Qualified Assessor or Designated Examiner and submitted with the endorsement application under 46 CFR 11.301(a).

When it’s used
Submitted with STCW endorsement applications whenever the standard of competence is demonstrated through in-service assessment rather than an approved course. Alternative company assessment documentation must be Coast Guard-approved before use.

The mariner performs the demonstrations; the Qualified Assessor records the outcome of each assessment on the control sheet.
Who signs
Coast Guard-approved Qualified Assessor (QA) or Designated Examiner (DE).
Where it goes
Include the completed assessment records with the mariner's STCW endorsement application to the National Maritime Center.
