A civil surgeon is a licensed medical doctor whom U.S. Citizenship and Immigration Services (USCIS) has designated to conduct immigration medical examinations for applicants inside the United States and to complete Form I-693. Civil surgeon designation is specific to immigration medical exams; a clinician who is not designated cannot sign Form I-693.
