Form I-693, Report of Immigration Medical Examination and Vaccination Record, is used by U.S. Citizenship and Immigration Services (USCIS) to determine whether an applicant for adjustment of status (or certain other benefits) is inadmissible on health-related grounds under INA §212(a)(1). For applicants inside the United States, the examination must be performed by a USCIS-designated civil surgeon. The civil surgeon seals the completed form in an envelope that the applicant submits to USCIS with the associated application.
