The U.S. Preventive Services Task Force (USPSTF) is an independent, volunteer panel of national experts in prevention and evidence-based medicine, convened and supported by the Agency for Healthcare Research and Quality (AHRQ). The Task Force systematically reviews the evidence on clinical preventive services — screening tests, counseling, and preventive medications — and issues recommendations graded A (high certainty of substantial net benefit), B (high certainty of moderate, or moderate certainty of substantial, net benefit), C (selective offering), D (do not provide), or I (insufficient evidence). Under the Affordable Care Act, most non-grandfathered health plans must cover Grade A and B services without cost-sharing. For occupational medicine, USPSTF recommendations frame the legitimate scope of "routine" prevention layered onto job-required exams — for example, the evidence (or lack thereof) for lipid screening, hypertension screening, depression screening, hepatitis C screening, lung cancer screening (for eligible heavy smokers), and tobacco-use counseling. Distinguishing USPSTF-recommended preventive content from job-specific surveillance (e.g., OSHA-mandated medical surveillance for silica or lead) is essential when designing pre-placement and periodic exam templates.
