Texas adopts the federal Medicare RBRVS system with Texas-specific conversion factors and ground rules under the Medical Fee Guideline (MFG). Separate guidelines cover professional services, hospital facility, ASC, dental, and pharmacy. Rates are recalculated annually based on Medicare changes.
