Washington is a monopolistic state fund. The Department of Labor & Industries publishes its own Medical Aid Rules and Fee Schedules covering professional services, hospital inpatient (DRG-based), outpatient (APC-based), ASC, and ancillary services. Rates are set by L&I and updated annually.

Note. Washington is one of four monopolistic state funds — private workers’ comp insurance is not permitted except for self-insured employers.
