    Provider completes the DWC PR-2 progress report after each visit and the PR-3 / PR-4 at MMI.

    Treating physician must be in the employer’s MPN (Medical Provider Network) unless the employee pre-designated.

  3. 3

    PR-2 is due to the claims administrator within 20 days of the exam (8 CCR §9785).
