What to Bring

Bring a valid photo ID (driver's license or passport)Bring your employer's order confirmation or authorization formBring: FAA MedXPress confirmation number (complete application at medxpress.faa.gov before your visit)Bring: Glasses, contacts, or hearing aids if you use themBring: Records of any hospitalizations or surgeries in the past 3 years

How to Prepare

If blood work is included, fast for 8–12 hours before your appointment (water is OK).Wear comfortable, loose-fitting clothing. You may need to change into a gown for parts of the exam.Bring a complete list of all prescription and over-the-counter medications you take, including dosages. Continue taking your regular medications unless your doctor advises otherwise.
