What to Bring

Bring a valid photo ID (driver's license or passport)Bring your employer's order confirmation or authorization formBring: Employer-provided hazmat exposure information or job descriptionBring: Previous hazmat physical results (if applicable)Bring: Respirator fit test results (if applicable)

How to Prepare

If blood work is included, fast for 8–12 hours before your appointment (water is OK).Wear comfortable, loose-fitting clothing. You may need to change into a gown for parts of the exam.Bring a complete list of all prescription and over-the-counter medications you take, including dosages. Continue taking your regular medications unless your doctor advises otherwise.
