Roaring Fork Family Practice has been part of the Carbondale community since 1975 and has grown to be a medical home to our population. By providing a medical home for our patients, our goal is to develop a relationship between members of all ages and health conditions. We specialize in pediatric care, newborn care, adolescent care, adult care, geriatric care, women's services, behavioral health, and more. We strive to be your first reliable contact for health concerns and can directly address most health care needs. Through commitment to coordinated care, we dedicate ourselves to help members navigate the health system by coordinating appropriate care and communicating with other qualified providers, specialists and health agencies in the community. Our medical home may not be a final destination, but thru primary care, we can help support that care is received in the right place, at the right time, and in the manner that best suits our member’s needs.
