Indiana does not maintain a statewide medical fee schedule for non-hospital services. Reimbursement for outpatient physician services follows usual and customary charges, subject to statutory caps and the negotiated rates between payers and providers. Hospital and ASC services are reimbursed at 200% of Medicare under Ind. Code §22-3-3-5.2.
