NOTE: *If coming from 690E or Route 81 S, follow signs to Route 481 North. *If coming from Route 81 North or Thruway I90, follow signs to 481 South. DIRECTIONS: Take Exit 5W off Route 481 and follow signs for Kirkville Road West (Cr-53). At stoplight (500 ft), turn right on to Fly Road. Travel north on Fly Road for approximately .2 miles and turn right in to parking lot for 6320 Fly Road. Look for the sign on right hand side that says MacKnight Bacon Professional Center. Passport Health is located on the second floor – Ste 214. There is an elevator located on the first floor at the north end of the building.
